I'm looking into secondary glazing for single pane windows in original wooden frames. Has anyone had this done and could recommend a company or any hints or tips?

We used this company. http://www.sashwindow.com/The system is called bi-glass. Because the space between the panes is smaller, noise is not totally eliminated but greatly reduced. Used them many years ago and has been trouble free since - no condensation and little heat loss.
