Arthur,I had an interesting conversation with a mental health professional recently. They indicated that the system itself may well be at the root of lack of integration, and even lack of care, given to those with mental health issues. They thought that the system effectively encourages GPs to hand off mental health cases to the mental health system and wash their hands of them. Unlike other health issues the GP ceases to be manager of services delivered to the patient, and the GP has little incentive to follow up or offer continuing care. As a result patients who are labelled as mental health cases get very poor general medical services and suffer poor health and early death as a result. It seems like the health system, which is now explicitly GP led through the CCGs, needs to change so that GPs own all their patients and their outcomes, irrespective of their mental health status. It also seems that Ealing CCG hasn't started to change its mindset yet!
